If you are searching for hair loss treatment in Sri Lanka, you will find pharmacies, Instagram resellers, herbal oils, salons, clinics and online doctors, all promising to fix thinning hair. Only a few options have strong evidence behind them. This guide is for Sri Lankan men with thinning at the hairline or crown. It explains what works, what is mostly marketing, how treatment is accessed here, and how to start without wasting a year.
The Quick Version
- Most hair loss in men is androgenetic alopecia (male pattern baldness). It is progressive, so it does not stop on its own [1].
- The best-supported medical treatments are 5% minoxidil and finasteride. They work best when started early [1][2][3].
- Biotin, herbal oils and most scalp “boosters” have weak or no evidence for male pattern hair loss [6].
- Finasteride tablets are sold as a prescription medicine in Sri Lanka. Minoxidil is widely sold without a consultation, but a doctor can confirm your diagnosis and adjust treatment.
- Judge any treatment after 4 to 6 months, not 4 weeks.
First, Work Out Which Kind of Hair Loss You Have
Not all hair loss is the same, and the treatments below only apply to one type. Here is how the common patterns differ:
| Pattern | What it looks like | Common triggers | What to do |
|---|---|---|---|
| Male pattern (androgenetic alopecia) | Receding temples, thinning crown, getting worse gradually over years | Genetics and the hormone DHT | Treatable. Start early. |
| Diffuse shedding (telogen effluvium) | More hair on the pillow and brush, all over the scalp, often a few months after a trigger | High fever, serious illness, major stress, crash dieting, some medicines | Often settles once the trigger is dealt with. See a doctor if it continues. |
| Patchy loss | Smooth, round bald patches | Often autoimmune (alopecia areata) | See a doctor. The treatments in this guide are not the answer. |
| Painful or scarring loss | Redness, pain, pus, scaling, sudden change | Infections and scarring conditions | See a doctor promptly. Do not self-treat. |
If your hair is shedding all over rather than receding in a pattern, a doctor may also check for causes such as thyroid or iron problems before starting any hair loss medicine.
Why Starting Early Matters
In men who are genetically sensitive, DHT gradually shrinks hair follicles over successive hair cycles. Each cycle grows a thinner, shorter hair until the follicle stops producing visible hair. Treatments are much better at keeping and thickening hairs that are still growing than at reviving follicles that shut down years ago. We explain the biology in The Science of Hair Follicle Miniaturization.
The Treatments With the Best Evidence
Minoxidil 5% (topical)
Minoxidil is a growth stimulant applied to the scalp. The European (S3) guideline for androgenetic alopecia recommends 5% minoxidil solution, 1 ml twice daily, for men over 18 with mild to moderate loss [1]. In a 48-week randomized trial, 5% minoxidil outperformed both 2% and placebo [2]. It only works while you keep using it. For the full picture, read our Medical Guide to topical minoxidil in Sri Lanka and the daily routine for Sri Lanka's heat.
Oral finasteride 1 mg
Finasteride blocks the enzyme that converts testosterone into DHT, which is the driver of male pattern loss. In the large trials, after two years about 66% of men with vertex (crown) loss showed improvement on photographic assessment versus 7% on placebo, and 83% had no further loss on hair counts versus 28% on placebo [3]. It is a prescription medicine with known side effects, which we cover in our finasteride guide for Sri Lanka.
Topical finasteride
Applied to the scalp instead of swallowed, topical finasteride aims to reduce DHT locally with less drug in the bloodstream. In a 24-week phase III trial of 458 men, a 0.25% topical finasteride spray produced hair count gains similar to oral finasteride, with plasma finasteride levels more than 100 times lower [5]. The honest caveats: the trial was only 24 weeks long, and the evidence base is smaller than for the oral tablet. See 0.25% Topical Finasteride: The Science.
Using them together
The European guideline notes that combining oral finasteride with minoxidil may give a better outcome than either alone [1]. A compounded product that combines topical finasteride with minoxidil relies on indirect evidence, meaning trials of each drug rather than one trial of the exact mixture, so it is best chosen with a doctor.
Hair Loss Treatments in Sri Lanka Compared
| Option | What it does | Evidence | Access in Sri Lanka |
|---|---|---|---|
| Minoxidil 5% solution | Stimulates growth, thickens hair | Strong | Sold by pharmacies and online retailers. See our price guide. |
| Finasteride 1 mg tablets | Lowers DHT, slows loss | Strong | Prescription medicine |
| Topical finasteride (compounded) | Lowers scalp DHT with less in the blood | Moderate (24-week phase III trial) | Doctor-prescribed, compounded |
| Ketoconazole 2% shampoo | Treats dandruff and scalp inflammation | Supports scalp health, not a regrowth treatment | Pharmacies |
| PRP, microneedling, laser devices | Procedures and devices aimed at stimulating follicles | Less consistent than the medicines above. Guideline authors have noted limited long-term safety data for low-level light therapy [1]. | Clinics and devices, prices vary |
| Hair transplant | Moves existing hair to thinning areas | Surgical option, does not stop ongoing loss | Clinics in Sri Lanka and abroad |
| Biotin and hair vitamins | Corrects deficiency only | Weak in healthy men [6] | Over the counter |
| Herbal and coconut-based oils | Scalp feel and conditioning | No strong evidence for regrowth | Everywhere |
What Does Not Work as Advertised
Biotin. A systematic review of the literature found that biotin helps hair mainly in people with a true biotin deficiency or specific rare conditions, and that evidence for supplements in healthy people is lacking [6]. It will not fix male pattern baldness.
Herbal oils. Oils may make hair feel better, but we have not seen good trials showing they reverse male pattern loss, and oil on a hot, sweaty scalp can worsen dandruff. Read Oil Is for Cooking.
Miracle bottles. Any product promising full regrowth in weeks, with no active drug and no doctor, is selling hope. Before-and-after photos taken in different lighting prove nothing.
Where Hair Transplants Fit
A transplant moves hair from the back of the scalp, which is resistant to DHT, to thinning areas. It does not stop the hair around those grafts from continuing to thin, which is why surgeons commonly pair surgery with medication. Waiting for a transplant instead of treating early usually means more loss to fix. We explain why in The Hair Transplant Fallacy.
How Men Get Treated in Sri Lanka
- Pharmacy or online retailer: Easy and fast for minoxidil. There is usually no diagnosis and no follow-up, and stock and prices vary a lot.
- In-person dermatologist or physician: A full scalp examination and a prescription for finasteride if appropriate. Expect waiting time and consultation fees.
- Online doctor consultation: A doctor reviews your history and photos, prescribes where appropriate, and follows up. It cannot replace an in-person exam when your hair loss looks atypical, and a good service will refer you on in that case.
Whichever route you choose, be careful with grey-market imports and social media resellers. Bottles you cannot trace to a source are a risk to your scalp and your wallet. See Brand vs Generic Differences for how to think about brands.
What Treatment Costs in Sri Lanka
In listings we reviewed in October 2026, 5% minoxidil 60 ml bottles from local pharmacies ranged from about Rs 3,435 to Rs 7,526, while ADAM lists 5% minoxidil at Rs 4,960 for one month with a doctor consultation included. ADAM's compounded minoxidil 5% plus finasteride 0.25% solution is listed at Rs 7,960 for one month. The cost that matters is the cost across 12 months of consistent use, which we break down in our minoxidil price guide.
A Realistic Timeline
- Months 0 to 3: Minoxidil can cause temporary extra shedding early on. Finasteride trials showed greater hair growth than placebo from about 3 months. Take baseline photos now. See the shedding phase explained.
- Months 3 to 6: Visible changes begin for many men. Leaflets note that 4 months or more of twice-daily minoxidil may be needed.
- Months 6 to 12 and beyond: The 48-week minoxidil trial and the 2-year and 5-year finasteride data are where results are measured [2][3][4]. These medicines keep working only while you keep using them.

Frequently Asked Questions
Can hair loss in men be reversed?
Male pattern hair loss can be slowed, and in many men partly reversed, if follicles are still active. Completely bald, smooth areas that have been that way for years are unlikely to respond. Earlier treatment gives better odds [1][3].
Do I need a prescription for minoxidil in Sri Lanka?
Many pharmacies and online retailers list 5% minoxidil without a consultation step. A doctor can still check whether it suits you and manage scalp irritation or shedding. Finasteride tablets are listed as prescription medicines.
How long before I see results?
Plan on at least 4 to 6 months. Early weeks can include shedding with minoxidil, which is expected and not a reason to stop.
What happens if I stop treatment?
Hair gained with these medicines is generally lost again over the following months, because they control the condition rather than cure it.
Is this guide for women too?
This guide is written for men. Finasteride is not for women, and pregnant women should not handle crushed or broken tablets. Women with hair loss should see a doctor.
